What are cookies?

What kind of cookies do we use?
Permanent – remain on the user’s device for the time specified in the parameters of cookies or until they are manually deleted by the user.
Session – remain on the user’s device until logging out of the website or turning off the software (web browser). We do not use this type of cookies as part of standard cookies
Analytical – used by integrated services, most often e.g. Google Analytics, including in particular website users, are subject to their own privacy policy

The personal data provided by the user in the on-line registration form will be used only to carry out the registration process and for the proper performance of the services available at Ortorad CLINIC Wojciech Radliński. Your personal data is processed in order to book a visit in accordance with your request. The legal basis for the processing of your personal data in this case is the necessity to process them in order to take action at your request before concluding the contract for the provision of health services (6 (1) (b) of the GDPR). Access to your data obtained from the on-line registration form is available to the Administrator’s staff and entities providing services to the Administrator (i.e. IT and technical support services, accounting services, external auditors and entities providing specialized services, e.g. legal advice,), which must have access to data to perform their duties. Your personal data obtained in the on-line registration process will be stored for a period consistent with the current requirements. You have the right to:
– request access to your personal data, rectification, deletion or limitation of processing, as well as the right to transfer data,
– if the basis for the processing of personal data is the legitimate interest of the Administrator – to object at any time to the processing of personal data for reasons related to your particular situation,
– lodging a complaint to the supervisory body, i.e. the President of the Personal Data Protection Office.
Your personal data will not be used to make automated decisions, including profiling
The personal data provided by the user via the contact form will be processed in order to answer your message. The legal basis for the processing of your personal data in this case is the Controller’s legitimate interest in handling your correspondence (6 (1) (f) of the GDPR). Our employees, subcontractors and entities providing services to us (i.e. IT and technical support services) will have access to your data, which must have access to the data in order to perform their duties. Your data will be stored for the period necessary to consider the inquiry. You have the right to:
– request access to your personal data, rectification, deletion or limitation of processing, as well as the right to transfer data,
– object at any time to the processing of your personal data for reasons related to your particular situation,
– lodging a complaint to the supervisory body, i.e. the President of the Personal Data Protection Office.
